Here is a shot of the roots. They seem to be winding around the bottom, and some of them look a little withered.

That mix is probably a bit too arid for typical Phalaenopsis growing conditions. Find some complete Phalaenopsis growing instructions on the net that relate to your particular zone 10. For what it is worth, I am growing mine in smaller size treated pine bark.

Your medium looks spent. Replant into a smaller pot. I use Schultz orchid mix and they do really well. Water when almost dry.
