Meyer Lemon woes

My Meyer Lemon is having some issues. Leaves have mottled spots and are dropping. The tree is fruiting, but it dropped a whole round of fruit after the first flowering and grew a few more after a smaller second flowering.
Could you help identify the problem? Image attached.
Better draining soil? More nitrogen? A disease?

Sarah, I suspect you've got spider mites. Treat accordingly (strong blast of water to wash off as many as possible. Treat with insecticidal soap. Repeat weekly for about 4 to 5 weeks. Watch for re-infestation and repeat the process if they come back.)
