Please help my identify the disease

I am very new to this and I am having a lot of success with cherry tomatoes and yellow monarchs however my Heirloom seems to be suffering. Can somebody identify my issue and recommend a treatment please?

It sort of looks like the beginning of Alternaria Canker.

But, could also be a nutrient deficiency, so use a good fertilizer labeled for tomatoes with all the micronutrients.
