Help to solve my problem !

This is the second time this year this Adenium has bloomed with the same blemishes .
I thought to much water but I keep these relatively dry.
so it can only be the soil or maybe when it was grafted it could have been infected in the Caudex
Any one out there have any ideas what the problem could be !!
