What's killing my onions?

For the second year in a row my onions have died just as they started to grow, the biggest bulb was only a little over an inch wide. All of a sudden the stems start to shred right at the base just above the ground. It happens pretty quickly once it hits a plant and it will eventually hit all the plants individually and kill them. It looks just like someone stuck a firecracker down into the stem and lit it, just shredded or blasted apart like that. If I leave the onion in the ground even after the damage it will get tunnels into it like theres a worm or larva eating it (but this could be just secondary invasion due to the primary damage).

I've googled onion diseases and I can't find any that seem to have this particular characteristic. I don't know if this is a nutrient, environment, pest, or disease problem. Does anyone have any suggestions?
