Recurrent fungal infection in fancy goldfish

I have a three or four year old fantail goldfish, pretty big, alone in a 29 gallon tank. For the past 6-7 months, he has been getting a fungal infection, the body kind that looks like a peeling sunburn, at least once a month--generally a week or so after the last dose of medication is gone. The tank has a heater (because he's a fancy) and the temperature is generally 70-72 degrees. I change 10 gallons of water once a week, and I have a Biowheel filter. I was treating him with the generic fungal medicine from Petsmart, which worked incredibly well for months and now doesn't work so well. Lately I've used sulfa, which worked great, and tetracycline, which didn't. When he's well, he's fabulously well, with lots of energy and shine and activity.

I'm sure I've trashed my water bacteria by now. I have no idea what to do, and I love my fish. Help?
