azalea died

It was doing great but I am pretty sure my soil is alkaline, my hydrangea is bright pink, so I added some acid amendment to it and it curled right up and died, but when I pulled it out it was really root bound, the roots looked just like they did when I planted it. I got it for Mother's day and immediatly planted it but the roots were bound up so I loosened them cutting the sides and pulling it apart from the bottom, but maybe not enough? Which was my biggest mistake?

sigh, I really loved that little plant too.
