Rhododendron dying, flowers dried on the plant

I have a nice-size rhododendron that I transplanted in the fall 2 years ago (2006) from an area under some trees to a new garder in which it is sort of the centerpiece. It overwintered fine and bloomed last year (2007) as well as this year (2008). However, I noticed this summer that the flowers had dried in place on the plant rather than falling to the ground, which I thought would be more normal, but I couldn't actually remember what they did in past years so I didn't really worry. Until now. At this point, the dried flowers are still there but there are a great many brown dead leaves under the plant and the whole plant looks very droopy, with the leaves sort of curled in on themselves and not shiny and perky the way rhododendrons normally are. The leaves on the plant are still green, however. What on earth is wrong with my poor plant?
