Sounds like I'm trying to disguise the fact that I have a problem -- It really is a friend's problem!

She is trying to decide whether to pull out this shrub. It has leaf growth only on the top. Lower leaves fell off. Even if we improve the soil and treat any disease/pest issues is it worth saving?
MY MAIN QUESTION: IF WE CUT IT WAY DOWN WE WILL HAVE NO LEAVES. WILL IT SPROUT NEW GROWTH?

Catmint, that isn't quite enough information. Was the shrub stressed by drought, poor drainage, is it in too much shade...are there specific symptoms that would make you suspect disease or insects?

A healthy rhododendron will usually respond to hard pruning but not this time of year in Z5 (are you in Z5?)....better would be in late Spring. If whatever is causing this one to look bad has to do with root damage, it may not resprout at all.
