Plant it now. It has been under a lot of stress but if the leaves aren't dry, it has a good chance of survival. Be sure where you are planting it is well-drained and gets at least a half day of sun. Feed it with Miracle Grow or some other food for acid-loving shrubs, and water it good, but don't over-water. Feed it again in the spring. I wouldn't trim it.

I may be a healthy deciduous azalea or a dying evergreen azalea. In any case, fall is a good time to plant azaleas. If it is dying, it is not a miraculous time, but a good time.

Miracle Grow is mostly nitrogen and that could cause the plant to break dormancy and produce tender shoots that would be frozen back and die. Fertilize in the spring when the plant shows signs of life with a good azalea fertilizer like HollyTone and again after the plant blooms. Use the fertilizer at half the rate recommended on the package.

* Miracle-Gro® Water Soluble Azalea, Camellia, Rhododendron Plant Food is a 30-10-10 fertilizer where the 30 is nitrogen. On the other hand HollyTone is a 4-6-4 fertilizer, much better suited to Azaleas.
