Did you extract honey from this particular frame before?

If so, you might have damaged the comb during the extracting process. If you have a tangential extractor, extracting all the honey from one side first, at too high a speed, could separate the comb from the foundation. The proper thing to do is extract half the honey from the first side, flip the frames and extract all the honey from the other side, then flip the frames again and extract the remaining honey from the first side.

My bet would be that you're using plastic foundation. If so, this is not uncommon.

There's a reason people still prefer beeswax foundation.
