Echeveria leaves started dropping

This echeveria was looking pretty good for a long time until one day a few weeks ago its leaves just started turning yellow and dropping. Now it's down to these tiny bud leaves and the stem is exposed - it looks like something happened to it but I'm not sure. Overwatering? Is the stem supposed to look like this - kind of deformed, looks like rotting in some areas, blackened up top where all the leaves fell? Thanks anyone for your help!

It would appear the stem of the plant in the foreground is rotted as it is shriveled. Unless there is some viable stem under the reining leaves, I would remove any leaves that are still good and try to grow new plants.
