It's not looking good, but it can certainly be salvaged.
Can you tell us any more? What is the sun/light exposure? Do you fertilize? How often are you actually watering?

I would recommend that you re-pot it into a mix that is mostly fine-grade Orchid bark (fir bark), with a pinch of perlite and a pinch of potting soil. This is very late in the season to be re-potting, but I'd worry that it won't survive the Winter otherwise.

Two weeks after re-potting, fertilize with a 1/8 to 1/4 strength dose.

from the pics of the soil I'd guess over watering. I agree with Josh on the soil mix. I grow my Thanksgiving cactus in pure orchid bark.

Thank you Josh and tommyr for replying! :) I never fertilise and it gets indirect sunlight for a few hours a day.... I will repot it today, but the only soil that I can find here (I live in Jordan and not sure where to buy it from) - I can find Peat Moss, Sand from the river and red soil which becomes muddy when you put water on it.... any ideas? :)

You COULD order orchid bark online. I wouldn't use the red soil if it gets muddy. Sand is good though but not alone.

In my opinion, none of those are options.
Unless the sand is incredibly coarse - 1/10 of an inch or larger - I would not use it. In fact, I wouldn't use sand at all...these plants, in my experience, don't prefer the weight/pressure of hard grit (sand, granite, quartz) around their roots - they prefer light and loose with the water running by and bringing in oxygen.

Can you buy "Orchid Mix" from a nursery or garden center?
