Looks like a star jasmine, maybe search "trachelospermum pruning" for more details. If need to make room for access you are stuck with cutting it back out of the way, easiest would be to take a power hedge shear and hedge it back. Leaf color is poor, effective fertilizing should also help it come back in more densely after pruning.

The thick top and thin bottom is the result of it growing up to the top and then being stopped there, this being a plant that wants to grow much bigger than the space that is being provided. To get a more even distribution of foliage now the planting would have to be cut down lower and re-started, with individual replacement stems being moved and tied into place to produce a fan-like distribution of branches.

When we moved here about 15 years ago a star Jasmine looked pretty much like yours. Not very healthy, the foliage sparse. It is at the base of a two story masonry wall. I cut it down to about 2 feet, installed a drip line to provide regular water. It recovered quickly, filled in well. I thought the concrete wall ugly and installed vertical wires to train the Jasmine up. It is now 15 to 20 feet up the wall and I trim it to keep it from covering a window on the second floor. I think if you cut yours down it would come back full, at least for a few years. Al

I would also do something about widening the path and making a large pad on ground level in front of the door. That way there would be a bit more room to avoid the jasmine and access the door. Just an idea. Min
