Return of the brown spots

My ventricosa is getting brown spots on its leaves again. it temporarily went away when I moved it outside, but it is coming back with a vengeance. This disease causes brown spots on the leaves, and make the lids open prematurely on the pitchers, resulting in deformed pitchers. can anyone tell me what is wrong with it?

that doesn;t help me much, because it stays cool enough, it might be humidity issues as I keep the plant well watered, but not very humid (the water does drain fine).
the two cuttings from it that I took last year, each has about 4 inch pitchers, are doing fine, but they are indoors in a south facing window.
