Hydrangea tree or shrub??

Hello all! I posted this to the hydrangea forum, but wanted to ask the regulars on this forum, too. This plant was grown from a cutting from my mother-in-law, who doesn't remember anything about the original plant. It's about 5 years old and has flowered for the last 3, but only had 3 or 4 blooms each time. The blooms are dome-shaped and greenish-white (I'm in zone 8 with very acidic soil). I'm pretty sure it's a hydrangea tree, but don't know if I'm supposed to prune it in order for the plant to establish a trunk; right now it's just a shrub and getting larger every year! I've got some links below that show the whole plant, a couple of close-ups and two close-ups of the trunk underneath all those branches (the second one is better).

Any details about what type hydrangea it is and how to best care for it are much appreciated!
