Dwarf lemon problems

Was wondering if anyone can help me out. We're in Peoria, Az (zone 8 I believe). We planted a dwarf lemon tree last March. It did really well at first, but within about six months or so, it started losing its leaves, and started looking a little sickly. I chalked it up to the extreme heat and just kept it watered and fertilized thinking it would come around. This year, no leaves again, it just starts sprouting blossoms that fall off almost immediately. No new stem/branch growth and two leaves that look pitiful. We bought some Vigoro Citrus/Avocado plant food and have used that once so far. No luck though. Everything else is growing great, roses, jasmine, and my vegetable garden is going to town. First time trying citrus so I'm lost. Any help would be greatly appreciated. Thanks!
