Meyer Lemon woes

Hello!
My Meyer Lemon is having some issues. Leaves have mottled spots and are dropping. The tree is fruiting, but it dropped a whole round of fruit after the first flowering and grew a few more after a smaller second flowering.
Could you help identify the problem? Image attached.
Better draining soil? More nitrogen? A disease?
