Fruit Cocktail tree, partially dying?

Hi,
We have a fruit cocktail tree since July of 2012 and it was a large 30 gallon tree when purchased. It has/had? a couple types of oranges, lemon, lime, tangerine and grapefruit. It had been doing great until recently when it seems that all but the original tree (grapefruit) have died (the grafts). I've attached some pictures, but Lucas Nursery where we purchased it was convinced it was some sort of rot, and they asked me to dig my finger into the roots under it and they said they would be soft. I can still get a 50% credit, but I'm not so sure its totally dead as the original stalk seems fine and even scratches a bit green. The green leafs seem fine, so can anyone offer any help? I'm in the Orlando area.
I take great care of the tree, I fertilize, insecticide, root stimulator, prune per Lucas instructions. I have more pictures I can email or with instructions can post here.
