Grafting questions

Please ask any of your own questions on grafting here, as well. Maybe it can be a nice little resource for people interested, such as myself.

Has anybody tried grafting two conifers in the ground, ala Axel Erlandson's circus trees? I'm specifically interested in picea pungens grafts.

Likewise, has anybody tried grafting to increase trunk girth (I'm thinking gingkos here), perhaps with approach grafting? I know bonsai folks do a lot of this sort of work, so perhaps they can chime in.
