Maybe English yew isn't hardy in Chicago, in which case the cultivar would presumably be of hybrid origin - if T. baccata and T. x media are the only two choices in this instance.

Otherwise you would study details of the foliage to try and figure out where it belongs.

I located the CBG plant database and they have it listed as taxus x media 'Wojtek'.

I have yet to find a US source.
