My Boston fern is dying!

I need help!

I was the (un?)lucky inheritor of a beuatifully lush and healthy Boston fern last year, and I brought it to my office. Unfortunately, in the eight or so months since I brought it in, it has been losing fronds pretty regularly, and is now a shadow of its former glorious self. I've been watering it once every three or so weeks very thoroughly, and once every two or three waterings, adding Miracle Gro to it. My office doesn't have the highest humidity usually, so I bought a humidifier three months ago that works very well, but it only slightly lessened the fern's devolution. It gets fairly direct light from overhead fluorescent lights, but is nowhere near any windows. Also, I've noticed that the root ball is slightly above the level of the soil by a few inches.

Is there anything I can do to at least bring it back toward a bit of health, if not back to its former glorious self? Any help would be very much appreciated. Thank you!
