Rotten tubers

I was dividing my tubers this week and noticed several clumps had tubers that felt soft. I gave one of them a firm squeeze and my thumb went right through it and into a gooey dark brown rotten mess. The outer wall of the tuber seemed to be in good shape and there was no visual indication of any problem. It seemed to have rotted from the inside out.
Did I do something wrong? I dug my tubers last weekend (ground was wet/damp), knocked most of the dirt off and allowed them to air dry for a few days. I divided them throughout the week as I got to them.
The tuber production in general was poor. Can I attribute poor production and rot to the cold, wet spring then blazing hot summer we had in central Ohio? Any advice would be appreciated.
Here is one of the tubers with rot:

RE: Rotten tubers

I've seen rot like this before, but only when I've dug in the spring. I haven't dug mine yet, I'm still cutting them down. When I'm done cutting, I'll start back at the first and dig those that I want. I hope I don't see this! My weather issue was not a hot summer . . . .
