exchange list editing question

I'd like to know how simply to update my list without wiping out the entire list? How does one do that? Before I only just add or delete an item and now it seems whenever I need to do an update, the original is wiped out and I've to repeat the entire list again.. So frustrating. Am I doing something wrong?
