Cutting Tips?

For my own learning, does anyone have any tips on growing cuttings, particularly at the stage where they have roots and the roots are beginning to hit the side of a cup in a partially closed clear tupperware and leaves are beginning to form. One by one I lost every single one as they would grow roots and just start to get leaves and then the roots would change from white to brown, the leaves would go limp and fall off and eventually the whole thing would die. I watched this happen to so many plants I'm really discouraged. It's not clear what I'm supposed to do once I get to this point of having roots and the first couple leaves and I have them in a cup?

So far I'm 3 for 66, that's not a good batting average. Is there a better method that is more forgiving than the classic improved baggie method that I've been using?
