We have a bunch of pampas grass about 3 feet in diameter at the ground. Do we cut it back every year or skip? How close to the ground should we cut?

Are you sure it's Pampas Grass (Cortaderia selloana)? I ask because you are outside it's hardiness range, and it's fairly common for folks to refer to all large ornamental grasses as Pampas grass.

Chances are, whatever kind of grass it is, it will need to be cut back to within 6" from the ground, before it's spring growth reaches 6".

Can you post a picture of what it looks like right now?
