What happened with my Blue Oat grass?

I had a really nice clump of Blue Oat Grass Helictotrichon sempervirens in my sunny garden for several years until finally one winter it never came back. I suspect root rot since that area can stay fairly moist all winter. So this past year I mounded up an area for it (did this for an ailing yucca in the same area and that worked great), got a new clump, and by late summer it had croaked. Looked browned out. I wasn't around to make sure it got well watered and we did have a somewhat dry summer, so I'm thinking that could have been the problem since it wasn't really established yet. I really like this grass and may get another but only if I can figure out what's going wrong.

What's weird is that we have another clump back in the wooded area that gets only mediocre sun and it also gets wet back there in the winter, but this clump seems to go on forever.
