Help me (re)identify this streak

I received the parent plant of this streaked Hosta in a swap a few years back. Unfortunately I misplaced the label and sender's name. I had gotten a small 'Korean Snow' at the same time.......I don't think this is 'Korean Snow'. Seems like it has unusual flowers and a Japanese(?) cultivar name. I have not had it set seed yet and also wondering if it is fertile. Could it be 'Hyuga Urajima'?? This is one of the earlier emergers here in NJ....most of my Hosta are still just emergin eyes. Thoughts??
