Asked a similar question not long ago. Here is the link. I use an ipad and android phone. Havent touched my actual computer or laptop in some time, so organizing isnt as easy as it could be. I know this isnt the spreadsheet you are looking for.

What I did was download evernote. I keep track of each individual hosta, two ways. This may not make sense unless you were to use the program. You have notebooks (folders) which organize your notes. One hosta note contains all the info and a photo from when the hosta was first purchased. The next note contains the hosta pics and garden location for the season. You can add tags that make it searchable by whatever you like. It also updates it on your phone, or other devices you download it to. Here's a pic of a typical note. The layout used is from the hallson web descriptions. If they dont have the hosta purchased, I still use the layout and add the info from other sites.

The bottom of that same note that contains the first pic of it planted. Also have a spot for how many eyes, width height and biggest leaf size that year. Havent filled that out yet. So far this system is working nicely.
