I grow my carnosa in a hanging pot and it's getting those long vines too. If you do put it on a trellis make sure you don't clip the tips downward, as they'll die off.
Many people use those orchid clips to hold their hoyas in place. Bread ties, yarn or even Velcro tape. It's this green tape sold for holding plants to their support.
My home-ec teacher has hers in a hanging basket (where I got my first cutting from). It was beautiful when it flowered! When I had Krimson Queen, it too, was in a hanging basket.

I prefer them hanging and just stick the vines back in to twine through the plant. If they start to get ungainly at the top, I just whack back the new growth and it will come in down lower. A lot of people do like the hoops, but it's better if you can start using them when the plant is very young. I imagine it would be hard to introduce it into a large plant.

You could certainly grow it on a hoop, but the mature stems are so rigid it might be a little difficult to manipulate it where you want it to go.
