Dead heads/dried blooms

Hi!

I have a beautiful, blue "blau doneau" Hydrangea. It's in a southwest window in our apartment in Massachusetts, in a nice large pot and has 5 flowering heads.

My question lies here: The blooms on the heads are drying up. Should I just clip off the entire head, or should I pull the dead blooms off one by one?

Also, how much should I fertilize this baby to get her to bloom again next year?
