Rockwool tomato seedlings dieoff

I've been having problems with tomato seedlings for my last couple of tries and I can't seem to find what's wrong.
Every time, after 1 week after sowing, my seedlings dryoff and die in around half a day's time. I have tried fertilizing more often, and also less often. The rockwool cubes and the tray have been sterilized.
On closer inspecting the stems, they become very thin and "tape"-like, as seen in the pictures attached.
Any ideas?
