I grow mine permanently in pots but what I would do is carefully slide the contents out (they may have some offsets) and tease them apart (you could try separating with a gentle spray of water), dig the new holes relatively deep (6" - 8"), and then carefully set each in their holes, partially burying the new stem growth. Fill with the soil that you dug, tamp, and water. I think the crucial part is to try not to break the sprouted stem or you'll lose the flowers for the year.
