moving want to take plants with me

This is my very first time posting.. Im moving to a new place, was wondering if I could take any of my plants with me (they come up every year) Oh also Im not that experienced with relocating plants once they are in the ground lol. I would love to take them with me. It is now Sept, is it okay to dig them up for the move?

| Most perennials can be dug and divided in September. You could dig now and put them in pots then take the pots to the new house. Two points to consider: Did the person buying your house agree to having you remove plants? Will the plants have enough time in their new garden to establish roots before the ground freezes? |

| If you can, water them well before you dig so they're nice and hydrated, and then water them more afterwards (but don't drown them), and then expect them to look like crap until they go dormant. If they're happy, they'll be growing new roots until the soil gets really cold. |
