Please help me save my willow

A BUNCH of pictures of my sick Willow tree ... you can see that the limbs are turning black/brown and look moldy in some pictures. Literally a week ago it was filled with bright green buds. Now I am afraid it is dying. Please help me save my willow tree!!

How about a little background? It sure doesn't look good. When was it planted? How did you plant it? What is the planting site like? Etc.
