Pest or Disease? (photo)

This has been going on for a couple of years. Can't figure it out. I've tried fungicides and bug-a-cides, nothing has worked. Superthrive has been keeping the plant growing and fighting, but I'd like to get rid of this plague.
Any suggestions? The photo, of course, is a blowup.
