do i need to cut off the foliage as well as keeping root ball intact?

No! Don't cut it back unless it's very large and there isn't enough root ball on it. The foliage helps feed the plant so you need it. Just dig as big a root ball as you can and move it intact and keep it well watered. It should do fine.
