Is this rose suffering from some sort of disease? see the photo, thanks.

I could be wrong but it looks like rose slugs. They look like small green caterpillars which hide on the underside of leaves.

I agree, rose slugs or sawfly larva. A good hard spray of water to the undersides of the leaves should knock them off. Or an insecticidal soap spray.
