Inherited a lovely yellow rose, please help identify

This rose was part of the package when we bought our first home two years ago. This rose was beautiful that first dry summer, however last year's wet summer did it in. I've never seen a plant drop leaves like that. Anyway, before ditching it, I wanted to know what it is so I could maybe get an idea of how to better care for it.

Thank you! I thought that it might be, but since my husband's name is Patrick I thought that was too big a coincidence.
