The rose looks like a hybrid tea or grandiflora, you can always prune back the blooms once they become spent, typically to a 5th leaf. I prune my roses hard in Feb. not more than a foot from the ground. I look for at least 3 lateral buds from the base of the plant. You can use a time release fertilizer but I would use this in spring when the weather is cool. Otherwise you can use a dilute with water fertilizer an fertilize every other week. Look for one specific for roses. I think your rose actually looks great for the time of year it is. Powdery mildew, black spot and aphids are typical for roses. Use a fungicide as a preventive once leaves emerge in spring and you will have less of a problem later in the season but it requires spraying every other week. It depends on how dedicated you are.
