Black and Blue Roots -- Something Wrong?

I posted this on the Garden Clinic forum, completely forgetting that there's a Salvia forum! Excuse the double-post -- need experts, I think. I have 2 Black and Blue salvias in different beds. Both were planted last summer. One has been up for about 3 weeks, no sign of the other one. Yesterday, I pulled on the dried crown, it was loose, I could see some other dried sections sticking up out of the dirt. So I dug it up and found these tuberous growths on the roots. I'm going to consider the plant dead, but I've had problems in this bed with plants dying for no apparent reason and have worried that I have some kind of soil fungus. Seeing these growths REALLY worried me! Is this how this plant is supposed to look?
