Dying Plumaria

My 15 yr. old plumaria is dying. The stalks (limbs) are mushy and completely black inside. It put out leaves this spring, then dropped them. What happened???

When is the last time you repotted?
Soil could be compacted and not draining well causing rot.
Cut off any good pieces and let dry then plant in a well draining soil.
