Tomato Stem Problem

I have been growing backyard tomatoes for about a year. Four months ago, they stopped growing past 5-6 inch stage. Often, the first inch of the stems seems to rot away the outter fuzzy core of the stem. It only happens in the first inch and then the plant seems to stop growing. Is this an insect of soil problem?

While not as common with such large seedlings, damping off (a fungal problem) can also occur, and will girdle the stem at the soil surface as you've shown/described.
