White flies (I think?) and Tiny green worms on tomato plants

Hi everyone, well I constantly use this website for tips so I figured why not post my own question since I am having so much trouble with this and really don't want all my hard work to go to waste!

I have two heirloom tomato plants in my back yard, they were doing great, until about a month ago when they got powdery mildew. I sprayed with neem oil and got rid of the problem. Everything looked good, they started producing fruit and then a few weeks ago I noticed holes in the leaves and started finding these tiny green worms on the underside of them. Now in the past few days the undersides of the leaves are covered in tiny white dots which I think are white flies? Just when you think you know what you're doing a new problem presents itself!

Anyways I have been spraying with a solution of neem oil and dish soap every three days and made some yellow sticky "traps" but nothing seems to be working! Please someone help me before Its too late! Thank you!
