bugs and low production

I have a beautiful grape tomato plant that has grown at least 3 feet high. It has only produced one tomato and it split before it was completely ripe. I am wondering why I only have one tomato. Is it the heat? It's been so very very hot lately. My plant buds all the time, but no tomatoes come of it.

In regard to bugs- I am not sure what these are, I have been looking all over the internet to tell me about these, but I just haven't found anything. The bugs leave black specks all over the leaves, then the leaves slowly die off. Entire branches then die. I have finally found the bug. I never saw it until today. It's a very small, long bug, with a yellowish horizontal stripe across it's body. I am attaching photos. Sorry the bug photo is so small. they really are tiny bugs.
