Plants Dying

This is our second planting of these plants. They did very well for weeks and were showing new growth when they started dying. There appears to be some kind of rot at the base of each leaf. Upon closer examination we see that some of them have no leaf problem, but the roots are gone. Thank you for aid given in this matter and if possible, an identification.
