Oak leaf disease?

I am trying to figure out what is happening to my oak tree. I'm not even sure what kind of oak it is - possibly black oak or pin oak. If it is anthracnose, what do I do to remedy it? I live in East Texas near Winnsboro. Any ideas would be greatly appreciated.
